We provide services statewide, ranging from 24 hours a day to several hours a week.
Qualified HWS staff will be trained and screened in accordance with the highest standards of the Bureau of Disabilities Services (BDS), in the State of Indiana. HWS will make sure that an individual’s goals are implemented in accordance with the most recent Person-Centered Individualized Support Plan (PCISP).

Service Notes
Residential Habilitation (RH10, RH20)
Residential Habilitation and Support services mean individually tailored supports that assist with the acquisition, retention or improvement in skills related to living in the community.
Examples: Assistance with personal care, meals, shopping, errands, chores and leisure activities, and transportation.
Exclusions: Services furnished to a minor by the parents, stepparents or legal guardian. Services furnished to an individual by the individual’s spouse.
Limitations: a paid (blood, adoptive or marriage) relative and/or legal guardian may not exceed a total of 40 hours per week per paid relative and/or legal guardian caregiver.
Transportation
Transportation services enable individuals to access non-medical community services, resources, destinations or places of employment, as well as maintain or improve their mobility within the community.
Limitations: Limited to Two one-way trips per day. Service will not be reimbursed when provided by the parent of a minor
Structured Family Caregiving (H&W)
Caregiving arrangement in which a participant lives with a principal caregiver who provides daily care and support to the participant based on the participant’s daily care needs.
Examples: Can be spouse or parent of minor child when on the H&W Waiver.
Limitations: HWS employes SFC caregivers as 1099 contract employees.
Day Habilitation (DHI)
Services are provided in a variety of settings in the community. Settings are nonresidential and separate from an individual’s private residence or other residential living arrangements.
Examples: Leisure Activities, Community Events, Educational Services, Hobbies, Volunteer opportunities, contact with family and friends.
Exclusions: Services furnished to a minor by the parents, stepparents or legal guardian. Services furnished to an individual by the individual’s spouse.
Participant Assistance & Care (PAC)
Services support and enable the individual in activities of daily living, self-care and mobility with hands-on assistance, prompting, reminders, support and monitoring needed to ensure the health and safety, of the individual.
Examples: Assistance with personal care, meals, shopping, errands, chores and leisure activities, mobility, correspondence and bill paying. General support and person-centered monitoring of the individual.
Exclusions: Services furnished to a minor by the parents, stepparents or legal guardian. Services furnished to an individual by the individual’s spouse.
Limitations: a paid (blood, adoptive or marriage) relative and/or legal guardian may not exceed a total of 40 hours per week per paid relative and/or legal guardian caregiver.
Respite (RSPO)
Services provided to individuals unable to care for themselves that are furnished on a short term basis because of the absence or need for relief of those persons who normally provide care for the individual.
Examples: Toileting, Feeding, assistance with daily living skills, grooming, personal hygiene, Meal prep, supervision/support, accessing the community.
Exclusions: Services provided to an individual living in a licensed facility-based setting. Other family members may not receive care or support from the provider while Respite is being provided/billed for the waiver individuals. Respite cannot be used as day/childcare. Respite cannot be used while the unpaid guardian attends school or work.
Remote Supports
Remote Supports includes a wide range of technology-based services that delivers support to an individual from a remote location in place of on-site staffing. Remote Supports provides a realistic, non-invasive way for many individuals and families to find the supports they need to build life skills and familiarity in their level of independence with a sense of security
Examples: Communication Devices, Visitor Safety, Overnight Supports, Fall Risk Mitigation, Kitchen Safety, Elopement Mitigation, Seizure Management.
Exclusions: Remote supports cannot be used in conjunction with Structured Family Caregiving.
